The following statements are provided in accordance with Wellbeing People's My Wellbeing People Account Privacy Policy and the Terms and Conditions.
I understand that my participation in the Habits for Health Programme and any coaching that I receive:
Is to inform and empower me to improve my wellbeing habits.
I am responsible for the actions and activities which I choose to engage in during and after the programme.
I am responsible for entering my own results.
It does not consist of any form of psychological therapy, medical consultation, or counselling.
If I am ever concerned about my health, I will seek advice from a healthcare professional such as a GP.
If I sign up to coaching, I will make every effort to give 48 hours' notice should I need to cancel a coaching appointment, rescheduling cannot be guaranteed.
Coaches are bound by confidentiality and will not directly or indirectly, use or disclose any personal information. The only exception being if the coach believes they have a duty of care, to notify the relevant authorities of a participant who is a threat to the safety of themselves, the coach, or a third party.
During group coaching I may obtain personal information about another participant and hereby agree that this is confidential, and I will not use or disclose this information.
My coach may take session notes, which will be anonymised and protected in accordance with GDPR policy, which is available on request.
Wellbeing People Ltd are committed to high coaching standards, so during some of my coaching sessions an assessor may join to ensure continued quality of training and delivery.
If I have any concerns, I will inform my coach as soon as possible or I will contact Wellbeing People via hello@wellbeingpeople.com or the 'Contact Us' page on the website.
